The office requires you to contact it for the cost before paying. Send the exact amount: shortages can delay the order, while excess payments are typically held in escrow instead of refunded.
Provide first name, last name and exact date of birth. Additional identifiers can help resolve common names.
DCI lists $15 for each last name searched, with a separate form and payment for each.
The online route requires a Request Form and Billing Form, payment information and a delivery destination. Specify how results should be sent.
DCI does not accept telephone requests. Immediate in-person results are limited to the subject presenting government-issued photo identification; requests about another person enter normal processing.
A signed release is not required just to request a check. Without it, DCI excludes completed deferred judgments and arrests older than 18 months without a final disposition. A “no history” response can reflect information that is not releasable. A release does not unlock confidential juvenile records.
